Fry Laboratories is not contracted with any insurance companies. Therefore, we do not complete insurance paperwork for our patients. However, our patients are mailed a sample 1500 Health Insurance Claim Form along with their paid invoices. This sample form is provided by Fry Laboratories as a courtesy to our patients to assist them in filling out claims on their own.
Payment for Testing
Our patients pay up-front and in full for their tests. Prepayment is necessary and must accompany the requisition form in order for testing to be completed. We accept Visa, Mastercard, or Check only. There will be a $25.00 fee for returned checks.

Invoices/Receipts
Paid invoices are mailed out every 2 weeks. Therefore, you should receive a paid invoice within 2 weeks from the date we received the specimen. If you do not receive the statement within this amount of time, feel free to contact us at (480)-292-8457 as it is possible the statement was lost in the mail or your address was not complete on your requisition form.
Cancellation Policy
The ordering physician is the only one allowed to request, change, or cancel tests. If you are thinking about canceling a test for any reason, your physician must submit the cancellation/change in writing to Fry Laboratories. We begin testing the specimen within several days of when we received it. Once testing has begun on the specimen, we cannot cancel the test and payment for services will be required.
